I hereby give to Behrman House (the “Publisher”) the right and permission, in respect of the attached photograph, to copyright the same, in the Publisher’s name; to reproduce, use, re-use, publish, and re-publish the same in whole or part, individually or in conjunction with other materials, in original or modified form in all media now or hereafter known, for the publishing, marketing, promotion, and/or public education activities of Publisher, and for any purpose whatsoever, including for illustration, promotion, advertising and trade.
I hereby release and discharge forever Publisher from any and all claims, demands, and causes of action arising out of or in connection with the above, including without limitation any and all claims for libel or invasion of privacy.
This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of the Publisher. This authorization and release shall be binding upon me and my heirs, legal representatives, and assigns.
I have read the foregoing and fully understand the contents thereof. I am the subject of the photograph and at least 21 years old, or I am the parent or legal guardian of the child pictured.
